Sign all forms with your First Name, Middle Initial, and Last Name. The name must match that shown on your Social Security Card. If your last name differs from that on your Social Security Card, please contact the Social Security Administration. Applications for new cards or replacement cards may be obtained through the Internet at: www.ssa.gov.
 
These forms, unless indicated otherwise, will be filed in your Electronic Official Personnel File (eOPF). Approximately three weeks after your start date, you will receive an email notification that documents have been added to your eOPF. When you log into eOPF you will be able to see/print your hire SF-50, Notification of Personnel Action and related documentation. Why: Used to identify Federal employers and periods of employment for which records must be obtained to verify the prior service.

How:

  • Required to submit even if no prior federal service.

Why: To credit such service for leave accrual rate and reduction-in-force purposes.

How: Applies only to military retirees. Complete address block and items 1-8. (Do not use this form if you completed 20 or more years of Reserve or National Guard service but will not receive a pension until age 60.)

How:

  • If you can't find the number of your degree write the name of your degree under the "Instructional Program" boxes.
  • Only include degrees you have completed, not those that you are still working on.

 

FMS 2231 Direct Deposit

Why : Whether on the road or at your duty station, this will ensure you receive your pay.

How:

  • Box 2 Type of Payment: Recommended to check all boxes.
  • Box 3 Routing # : All numbers may not fit in the boxes. You may add all numbers on form.
    A voided check may be used in lieu of completing this section.
  • Section 4: Allotment Information: Allotments are a set amount of money you would like to go to a specific bank account, such as a savings account.
  • If you would like any, you must fill out Box 4 Allotment Information.
  • If you want more than one, fill out additional Direct Deposit forms

How: Complete per form instructions. If you need separate withholding for State Income tax purposes you must complete 2 forms, identifying one for State and one for Federal.

Change in Job Standards - Residency Requirements

Why : Required to have a domicile that is within 1 hour or less commuting time, under normal weather and road conditions, to the employee's duty station headquarters.

How:

  • Hourly Employees only
  • "Domicile" is the place where an employee has his/her permanent home or residence.

 

FEHB PC Waiver

Why : This form is used to elect or waive pre-tax treatment of employee premium contributions to the FEHB Program. Pre-tax treatment is automatic.

Federal Employees Government Life Insurance FEGLI

Life Insurance Election - Automatic Basic Coverage unless waived

Why:

  • Basic coverage is effective from the first day you enter pay and duty status.
  • If you waive coverage and return the form before the end of the first pay period no premiums will be deducted.

How:

  • Forms must be completed and returned to Employee Services within 60 days (or the date you first become eligible) of your appointment date.
  • If you do not return the form you will continue to be covered by basic life insurance and charged for it.

TSP-1 Thrift Savings Plan Election Form

* Thrift Savings Plan Election Form Why :
  • Use this form to start, stop, or change the amount of your contributions to the Thrift Savings Plan (TSP), the voluntary retirement savings and investment plan for employees under the Federal Employees' Retirement System (FERS) and Civil Service Retirement System (CSRS).

How:

  • You may start, stop, or change contributions at any time.
  • Your contributions will change the beginning of the pay period after the form is received.

  • Employees who are older than age 50 may make additional "catch-up" contributions.

Designation of Beneficiary

Designation of Beneficiary - Unpaid Compensation of Deceased Federal Employee

Why:

  • Provides information regarding election of options for unpaid compensation, life insurance, retirement annuity and Thrift Savings Plan.


How:
  • You must file a new form if you make different decisions regarding those designations. Transfer employees need to complete new designations for unpaid compensation when changing agencies.

     

  • Your signature must be witnessed if forms are signed prior to New Employees Orientation.
